Project Request Web Page

Academic or administrative units can request projects either using the faxable form or our on-line request form. Both can be found under the Guidelines.

Guidelines:

Most requests are to renovate, reconfigure or refurbish existing space, space that usually is within area currently used by the user group. In some cases a request will necessitate new space, either in the form of new construction or space not currently in use by the department.  It is important for Facilities to understand this aspect of your request.   The following general information should be provided for any project request. An asterisk (*) indicates that the field is required.


  • * Subject: Insert building name and room number, or area of building affected by the proposed work. 
  • * Description: A short description of the desired work; for example: Paint and re-carpet space, build new pantry with sink in existing closet.  Or Renovate existing lab for Professor Smith.   Try to be as descriptive as possible, so we get a good sense of the magnitude of the work. 
  • Attachments: Documents can be attached, such as program or diagrams of space. 
  • * Project Requestor’s name: Insert your name. 
  • * Project Requestor’s Email: Insert your email, which will enable us to communicate with you on regarding your project request.
  • *Academic Chair or Authorized Administrative Manager: Please provide the name and email of the academic chair, in the case of an academic department, or authorized administrative manager, in the case of an administrative unit.  This may be a vice president, director or manager, but the person must be authorized to fund the project cost.  This will help to confirm the project aligns with the Department’s overall goals, and enable us to copy the authorized individual on follow up emails. 
  • * Project Request Type: Select project type from list provided (Study, Furnishings, Landscape, Renovation, etc.)
  • Project Request Type Other: If you cannot find a suitable Project Request Type; select Other, then enter the Request Type in this field.
  • * Project Space Type: Select project space type from list provided (Classroom, Office, etc.) 
  • Project Space Type Other: If you do not find an appropriate space type in the list; then select Other, and enter the space type in this field
  • * Budget Limit: Although we acknowledge we’ll need to provide a cost estimate of the proposed work, please provide any parameters of budget; your estimate of work, or more importantly any budget limitations.
  • * Project Funding Source: Please select appropriate anticipated project funding source (Departmental, Capital Plan, etc)
  • Project Funding Source Other: If you cannot find an appropriate funding source in the list; select Other, then use this field to enter the funding source.
  • * Project Schedule: Please insert approximate timeframe you desire the project or renovation to be completed (August, end of summer, winter break).
