Which Service Form Should I Use?
Maintenance
Work Request (University account holders)
Use this form to request general maintenance such as plumbing, custodial, electrical, carpentry, exterminator, elevator, heating and cooling, grounds maintenance and locksmith services in the building in which you live or work. Laundry equipment repair service for off campus laundry facilities are also requested on this form. You may also use this link to check the status of a previously submitted work order.
Laundry Equipment Service Request (Dormitories only)
Use this form if the washer or dryer needing repair is located in any undergraduate dormitory laundry room or at the Graduate College. Submittal of this form will report the problem directly to Mac-Gray Corporation manager of laundry services on campus. You will need your netID to access the form. If you are reporting a problem with a washing machine or dryer in off campus housing complete a work request form instead.

Project Initiation Request Form
Use this form to initiate a renovation or refurbishing project meeting these guidelines.
Equipment Rental Order Form
Use this form to order equipment such as tables, chairs, podiums and more for University events held on campus and to request setup and breakdown of equipment as needed.
Moving and Storage Service Forms
Use this e-mail address to request routine moving service of office equipment and furniture. Use this form to request bulk materials storage in the PU Storage Facility. Use this form to request records storage at the PU Storage Facility.
Surplus System
Use these links to access the online Surplus System. If you have a University ID, you can submit, view, and purchase items in surplus. The public can view items in surplus.
Other
Vending Machine Refund Form
Use this form to request a refund from a malfunctioning vending machine. Note that the refund limit is $3.00 and that you will need your netID and password to receive your refund.
