Upon completion of this session, the participant shall be able to:
1. Recognize gaps in communication during end of life care
2. Identify important clinical features that can improve prognostication accuracy.
3. Utilize the NJ POLST form as a tool to organize patient care.
There is no cost for the conference and CME and CEU credits will be mailed to participants after completion of evaluations.
