Dependent Verification

The University’s Benefits Committee asked that the Office of Human Resources verify the eligibility of each faculty and staff member’s dependents enrolled in Princeton’s healthcare plans, including medical, dental and vision benefits, prior to the completion of the 2010 Open Enrollment Period. (If you are not currently insuring dependents, this announcement does not apply to you.) This audit is a cost savings measure that will enable us to continue to provide optimum benefits for minimum cost. It will also ensure that the University is compliant with federal regulations.

To verify your dependent’s status, you must provide specific documentation to a Benefits staff member anytime now through the end of Open Enrollment, October 30, 2009, for each dependent enrolled in one or more of Princeton's healthcare plans and for whom you wish to continue coverage. To avoid a delay during the 2010 Benefits Open Enrollment Period, we must receive your documentation no later than October 30, 2009. If we are unable to verify your dependent’s eligibility, your dependent will not be covered as of January 1, 2010. Please refer to the information that describes the definitions of eligible and ineligible dependents and a list of the types of documentation that we will require. All documentation received will be handled confidentially.

Gathering the necessary documentation may take time. We, therefore, encourage you to avoid waiting until the last minute. As soon as you have the documentation available, please contact the Benefits Team at 258-3302 or at benefits@princeton.edu to make arrangements to come to HR. In addition, we will schedule “office hours” at various departments to make the verification process easier for you. During this time, Benefits staff members will be available to answer questions and review and collect required documentation. Contact us if you would like us to come to your department.

Faculty and staff members can now verify that their dependent verification documentation has been processed.

To log onto HR Self Service and review your dependent’s data, please follow these steps:

1.    Go to the HR Self Service website.

2.    Click the Log In option on the side menu bar.

3.    Enter your netID and password and click the Sign In button.

4.    Select Self Service and then select Benefits and Dependent/Beneficiary Info

5.    Look for the “Dep. Cert” check box in the column next to “Date of Birth”. For each dependent there is a “Dep. Cert” checkbox. The checkbox will be checked when your dependent’s data is up–to–date and eligibility for coverage has been verified.
