How the Plan Works

Use an In-Network Vision Care Provider

  1. Get a current directory of vision care providers, visit MetLife’s website at www.metlife.com/insurance/vision-insurance/ and choose the MetLife Vision PPO or call 855-MET-EYE1 (855-638-3931) and ask about Plan MC0011.
  2. Schedule an appointment.
  3. Bring your ID card to your appointment. If you do not have your card, you can call MetLife at 855-638-3931 for a replacement, or a temporary ID card may be printed from MetLife’s website at www.metlife.com/mybenefits.
Reimbursement for Out-of-network Provider
 
There are two ways in which an out-of-network claim may be submitted to MetLife. 
  1. Check with the out-of-network provider to see if they will submit the claim electronically to   MetLife. If the provider will submit the claim, you should only need to pay for your portion of the services rendered per the reimbursement schedule.
  2. Pay for the full amount of the services, and submit a claim form to MetLife to receive your eligible reimbursement. You can also get the claim form by calling 855-MET-EYE1 (855-638-3931). Complete the claim form and attach the itemized receipt from the provider and mail to: MetLife Vision, PO Box 385018, Birmingham, AL 35238-5018.
