Key Dates

• Faculty and Staff will have mailed to their home address an Open Enrollment package which will include the Open Enrollment Guide.

• Main Campus – 1 New South, E-Quad, Firestone Library, Macmillan Building, 87 Prospect (OIT), Jadwin Gym, Lewis Thomas Lab
• Princeton Plasma Physics Laboratory – Lyman Spitzer Building, Human Resources Division

• Representatives from the health care plans and the Office of Human Resources will be available to provide materials and answer questions.

• All benefit enrollment changes must be submitted online by midnight; however, there is no HR assistance after 5:00 p.m.
• If you have requested a paper enrollment form, you must submit this enrollment form so it is received in Human Resources no later than 5:00 p.m.
• Please note: If an election is made online and via paper, the online election will prevail and the paper form will not be processed.

Confirmation Package Mailed

• Opportunity to review and/or make changes to Open Enrollment benefit elections via eBenefits. All changes must be made by Friday, December 4, 2009.
