OIT employee and consultant confidentiality acknowledgement form

Departments that have employees or consultants who have access to confidential or sensitive information should have a confidentiality form, tailored to the department's needs, that is signed by any individual who has access to such information.  The purpose of such a form would be to remind the signer that the information being handled is sensitive, and to convey the individual's responsibilities in protecting that information.  The following is the form being used by OIT that departments can use as the basis for their own form.
