For Feedback - Please fill out our Customer Service Form Services To facilitate resolving problems, an online form is provided so students can describe their concern or feedback. Manual reporting forms are also available at the Frist Campus Center Room 118 and at New South Building, B Floor. When there is a problem it is important to isolate the cause. The best way to resolve a problem is by supporting the documentator with specific details, e.g. an envelope postmarked in Ohio on the 13th and delivered on the 29th. Suggestions to improve service are always welcomed. Each suggestion or concern will receive a written response. |
