Please complete the order form, noting the author, title, year, page count, and, if applicable, the thesis number.
Calculate the total charge of your order, noting that the charge for photoduplicating material in our collections is thirty-five cents per page plus the following postage charges: Domestic Postage: 1-100 pages: $6.00; 101-500 pages: $12.00; 501-1,000 pages: $18 [International Postage: 1-100 pages: $12.00; 101-250 pages: $20.00; 251-500 pages: $35.00. For orders of 501-1,000 pages, additional shipping charges will apply. Please consult an archivist for further details.]. Postage fees will be waived for PDF scans, which are delivered electronically.

Sign and date the form, and return it to the Seeley G. Mudd Manuscript Library, Princeton University, 65 Olden Street, Princeton, NJ, 08540, with a check or money order made payable to the Princeton University Library. By signing the form, you agree to the copyright restrictions described therein.
If you wish to pay by credit card, please email a copy of the completed order form to the Mudd Library, and we will send you an electronic invoice. Please include your credit card billing address in your email, and, if different, your shipping address. Please do not send your credit card information.
All orders must be prepaid and are processed in order of receipt, usually within two to three weeks. Failure to sign the form or return it with your payment will result in a delay in processing.
