Forms
The coordinator of your program will give these forms to you to be filled out and signed by both you and an adult, and returned back to them prior to your visit to Princeton-Blairstown Center. If you need another copy, please select the proper form, print it, fill it out and give it to your trip coordinator. You will not be able to participate in any activities unless we have received properly signed forms prior to your program.

The following are definitions for the different groups that utilize our facilities to help you decide which forms you will need:
What is a Chaperoned Group?
A group that is accompanied by a parent or teacher who remains with the group for the duration of their stay and is responsible for the participants overnight and at mealtime. Please see our list of chaperone responsibilities.
What is an Unchaperoned Group?
A group that has no adult chaperones with them. PBC staff members are responsible for the participants 24/7. If you want to explore this option, please contact Heather Haynes at (908) 362-6765.
