Please find, print, and completely fill out the form specific to your group (see below for definitions), and return these forms via traceable mail to the Blairstown Campus at least one week prior to your program. Please note that activities cannot start until we have had a chance to review the forms. Be sure that all forms have the proper signatures on them before returning to us.
Any forms received late can be faxed to PBC at: (908) 362-7699. If any forms are brought with you on the day of the program, the entire group will need to wait until PBC staff have reviewed them.

The following are definitions for the types of groups that utilize our services to help you decide which forms above you need to use:
What is a Chaperoned Group?
A group that is accompanied by a parent or teacher who remains with the group for the duration of their stay and is responsible for the participants overnight and at mealtime. Please see our list of chaperone responsibilities.
What is an Unchaperoned Group?
A group that has no adult chaperones with them. PBC staff members are responsible for the participants 24/7. If you want to explore this option, please contact us at (908) 362-6765.
