All undergraduates who wish to enroll in a graduate course must complete this form and obtain the permission of the instructor, departmental representative (in Religion or the relevant department), and residential college dean.

If you wish to have a non-departmental course counted as a cognate, please complete this form, attach a copy of the course syllabus, and submit it to the Religion department office for review by the departmental representative.�Please note that, in order to be approved, cognate courses must be related to your area of focus within the major.
