Information for Departments Sponsoring Undergraduate Travel
Undergraduate Travel Requirements
Departments that sponsor undergraduate travel are reminded that students must:
- Register their travel on the travel database prior to receiving funding
- Contact UHS/Travel Medicine and make an appointment well in advance of departure if traveling to a country which requires immunizations
- Review thoroughly the information on the travel website for undergraduate students
- The University does not sponsor undergraduate travel to countries on the US State Department travel warning list. Students who have a compelling academic reason for traveling to a country on the State Department travel warning list may request consideration through the travel exemption process.

Step by Step guide
- Terms and Conditions form (on-line certification)
- Medical Profile (down load complete, student carries it with them while traveling)
- Read the SOS information on the travel website (on-line certification)
- Obtain an SOS card from Risk Management, or Department Manager (to be carried while traveling)
- Register international travel with the US State Department Smart Traveler Program
- Complete contact information and emergency contact information (while traveling)
- Detail transportation plans (dates, times, carrier, etc.)
- Detail lodging information (name, contact information)
Travel database acknowledgement and release of funds
- Funds may be released once the preliminary registration is complete
- Date and location of travel
- Travelers complete registration process w/exception of transportation and housing information
- Trip may be acknowledged once transportation and housing information entered
