Welcome Incoming Graduate Students

Please allow yourself sufficient time to obtain your immunization record and any required immunizations from your healthcare provider. UHS suggests you obtain a physical examination prior to matriculation. Please make an appointment with your healthcare provider now. An immunization hold will be applied to your academic registration until the completed Medical Information Form with immunizations is on file with University Health Services.

Dependant and Family Coverage

Dependent Enrollment Form 2009-2010
A separate fee is charged for dependents in the Student Health Plan (SHP). Open enrollment occurs twice each year.
