Return to Work Evaluation
Faculty and staff who need to be absent from work as the result of an illness or injury which lasts for 8 or more consecutive calendar days must visit Employee Health Services for evaluation before returning to work. Faculty and staff must bring their completed medical certificate and Application for Temporary Disability Benefits Form (PDF) to the evaluation if it was not submitted prior to the return to work date. Faculty and staff can get these forms from their supervisor or department administrator or at the Human Resources web site.
