It is important that you allow yourself sufficient time to get a physical examination and your immunization record from your healthcare provider. If you have not already made an appointment with your healthcare provider, do so now.

NCAA Medical Exception Form For ADHD/ADD If you are taking a stimulant medication (i.e. adderall, concerta, Ritalin) for treatment of Attention Deficit Hyperactivity Disorder (ADHD), print the form and have your prescribing physician complete it. Bring it to your sports physical on campus. |
