SHP Waiver/Enrollment (required) Compare your healthcare coverage to the Student Health Plan (SHP) and decide whether you will enroll in the SHP or waive. Log in to MyUHS and complete the Waiver/Enrollment process by selecting Forms from the navigation area on the left of the screen.

Sickle Cell Testing (required) Per NCAA recommendation, Princeton requires that all student athletes, prior to any practice or strength conditioning, be tested for Sickle Cell Trait (SCT). Order this test when you visit your doctor and upload a copy of the lab results to MyUHS. |

NCAA Medical Exception Form For ADHD/ADD (required for varsity athletes) If you are taking a stimulant medication (i.e. adderall, concerta, Ritalin) for treatment of Attention Deficit Hyperactivity Disorder (ADHD), print the form and have your prescribing physician complete it. Upload the form to MyUHS. |
