Optional Health Benefits – Vision And Dental Plans
ELIGIBILITY
All enrolled undergraduate and graduate students (including DCE students) and their dependents are eligible for participation in the optional vision and dental plans.
ENROLLMENT
Open enrollment will beginin early May (see e-mail notification from the Graduate School and University Health Services) and run through mid August. To enroll in one or both plans, the student must complete the dental enrollment form and/or the vision enrollment form and submit the completed form(s) to the SHP office by the August deadline. Send no payment. Your Student Account will be billed accordingly. For returning students, enrollment must be completed every year if you wish to continue your coverage. Coverage is NOT automatic from one year to the next.
DENTAL PLAN
The optional dental plan is administered by Healthplex/Eastern Dental. Enrollment will be accepted from May 1, 2009 through August 14, 2009.
VISION PLAN
The optional vision plan is administered by Vision Service Plan. Enrollment will be accepted from May 1, 2009 through August 14, 2009.
