TIGER CAMP PROGRAM OPERATION Option A: Friday: 12pm until midnight (lunch & dinner served) + Saturday: 5pm until midnight (dinner served) @ $130. Carnival included. Option B: Friday, 5pm until midnight (dinner served) @ $60. Option C: Saturday, 5pm until midnight (dinner served) @ $60. Parents may check their child(ren) in or out of the program at any time, but for safety reasons parents must sign child(ren) in and out of the program. Parents check their children in at age-appropriate campus sites, NOT at the YWCA Princeton. You will receive a postcard confirming your registration and indicating the site locations. Ages are separated as follows: Infants and Toddlers (0-3) Pre-school (3-5) School Age Children (Kindergarten-13) SCHEDULING To help us with our staffing and scheduling demands, PLEASE give us your best estimate of your child’s participation in the program. Dinner__ All children must be admitted to the program by 8:00pm both Friday and Saturday night, unless prior arrangements have been made. Registrations post-marked later than May 15 will incur a $20 late fee per family. We are sorry, but we are unable to provide refunds to any registrations received after May 1, 2001. The YWCA Princeton Tiger Camp welcomes children from 3 months to 13 years of age to join our child care program for the Princeton University Reunions. The emphasis is always on safety, fun, enrichment and providing a memorable experience for the children, while parents can enjoy the reunion festivities. The campers will be organized by age at three locations which are located on the Princeton campus. All of the campers will be supervised by experienced childcare providers, which already work for the YWCA Early Childhood Education or Athletic Departments. All of our staff have had criminal background checks and the majority of individuals are CPR/First Aid certified. The Tiger Camp provides the campers with various games, activities, and special events while in our care. For example, the children will all have an opportunity to play various sports activities, do Arts&Crafts, play group games, juggling, computers (if available), Carnival, Magician, Game Show, and much, much more! Release, Waiver and Consent FormI am the parent/legal guardian of ____________________________ who is, with my permission, a "Participant" in the Tiger Camp Program, sponsored by The YWCA Princeton, Princeton University, and the individual Reunion Classes of Princeton University, for mybenefit during the Princeton Reunion Weekend, June 1 and 2, 2001. In the event that I am not immediately available, should the Participant suffer a serious or life-threatening injury for which emergency medical treatment may be necessary, I hereby authorize an appropriate adult staff member, designated by the Board of Directors of The YWCA Princeton, to engage qualified medical personnel to initiate any necessary medical treatment or care. In the event of such an injury, it is understood that The YWCA Princeton will use all reasonable efforts to notify me (or the emergency contact listed on my child’s application), where practical, prior to initiating medical treatment for any such injury to the Participant. Should neither party be available, an appropriate staff person will contact appropriate medical personnel to initiate the necessary medical treatment, and I hereby give permission to any such physician or other medical personnel to provide such medical treatment such individual deems medically appropriate. I agree that medical treatment for any other type of injury may be coordinated by the YWCA in consultation with appropriate medical personnel. I understand and agree that I am responsible for all medical care expenses incurred to treat the Participant’s injuries including, without limitation, physician, hospital, lab, drug and device expenses. The following policies or coverage are available to cover the cost of medical care to treat any injury incurred by the Participant: Insurance Company______________________________________________ Policy #______________________ My child has my permission to be transported or to walk, under supervision with Tiger Camp staff, to other Princeton University facilities: On behalf of the Participant, the Participant’s parents, and/or legal guardians, I hereby give approval of the above-named Participant’s participation in any and all programs and activities sponsored or provided by the YWCA Princeton, Princeton University, and the Individual Princeton University Classes in connection with the Princeton Reunions Weekend of June 1 and 2, 2001, and do hereby waive, release, absolve, forever discharge, and agree to hold harmless the organizers, supervisors, participants, and persons involved in the operation, organization, sponsorship, supervision or participation of these activities and programs, including without limitation, The YWCA Princeton, Princeton University, the Individual Princeton University Classes, and all their respective trustees, directors, members, officers, employees, alumni, agents, students, Reunion Committee persons, contractors and subcontractors, for , from, and against any claim or cause of action of any nature whatsoever that may be available to the Participant or his/her parents and/or legal guardians, arising out of any injury, accident or illness to the Participant, arising in any way out of or in connection with the Participant’s participation in such programs and activities. ____________________________________________________________ ________________________ © 2000 The Trustees of Princeton University Last Update: October 16, 2000 |
