Regarding an author who is referred to as "xxx le-mishpahat yyy" (e.g., Eliyahu ben Aryeh Leyb le-mishpahat Binkovitser, mi-Kobrin):

It  may  mean  that Binkovitser is  the surname, but  it  may also be  a name in  the mother's family or just the name  in  the whole lineage that he's proudest of.   As a result,  the heading should be "Eliyahu ben Aryeh Leyb, mi-Kobrin.",
and not "Binkovitser, Eliyahu ben Aryeh Leyb.
