"Making Up" a Name

We occasionally have distinguished identical names when the author's father's name has been given in a patronymic elsewhere than the chief source--that is, we have "Mosheh Levi" in the chief source and "Mosheh ben Yosef Levi" elsewhere, so we make the heading "Levi, Moshheh$q(Mosheh ben Yosef)."

Can we do that if we know the father's name, but it doesn't actually occur WITH the son's name?  Suppose Mosheh Levi's book is dedicated to "my father, Yosef Levi" but the form "Mosheh ben Yosef Levi" doesn't actually occur?

CPSO: No, we don't think that "making up" a name is legitimate, even to resolve a  conflict.

Since the answer is no, one cannot qualify the name as he suggested,and  it looks as if Mosheh Levi will have to become an undifferentiated personal name (= non-unique).
