Author's Name not in the Work's Language

How to establish a name of an author, whose  work  is  in Hebrew, but the author's name  does  not  appear in Hebrew on  the  title page,  although  the  t.p.  says  that it  is spelled out by the initial letters of the Hebrew introductory poem, which is *not* on  the  t.p.   The author's name appears on the t.p. in Cyrillic.

The *book* is in the author's language, per rule 22.1B,  but  the  t.p.  form  of   the  name  isn't.   Must  he   be established, provisionally, from the Cyrillic form?  Or  may  we  use  the Hebrew form (buried in  the text) and  code  it provisional?

According to CPSO, one should  go  with  the  form  on  the  t.p.,  unless  the  author  is  well-known or  has other publications than this, and code  it as provisional.

This means that "Khanania Lipa" will be  the heading, and  it  will be coded provisional.
The first ref will be  the  form from  the acrostic, which seems to be "Hananyah Lipa ben Natan Neta'"
Then there will be  a  ref  from "Lipa, Khanania," and probably "Lipa,  Hananyah" (though a variant of  a variant) would not  be  out  of  place.    In  the  670,  one could say  something  like:   "t.p.  (Khanania Lipa  (in Cyr.)) akdamut (Hananyah Lipa b.m.h. ve-r. R.  Natan Neta' (in  an acrostic))"-- and then whatever other sources one wants to cite.
