Student Registration - Undergraduates

This form should be completed by Undergraduate Students who wish to self-identify a disability and request academic accommodations from the Office of Disability Services. 

Student Registration - Graduate Students

This form should be completed by Graduate Students who wish to self-identify a disability and request academic accommodations from the Office of Disability Services.

Graduate Student Disability and/or Medical Accommodation Request for Housing Form

Graduate Students should complete this form when requesting special needs for housing.

Disabilities and Other Special Needs form - Incoming Freshmen

Incoming Freshmen requesting housing or academic accommodations should complete this form, that is also available through the on-line matriculation packet.  This form should be mailed to the Office of Disability Services along with appropriate documentation of the disability. 

Request for Temporary Academic Accommodations

Students who have a temporary impairment should complete this form and submit it to the Office of Disability Services when they are requesting academic accommodations.

Request for Initial Determination of Academic Accommodations for Undergraduate Students with Disabilities


Admitted undergraduate students with disabilities must submit this form to request an initial determination of academic accommodations to ODS on or before April 5th.

Release of Information from ODS

This form should be completed by students who need the Office of Disability Services to release student information to a third party.  Please complete this form and return to the Office of Disability Services.

Release of Information to ODS

In evaluating a student's documentation or disability, the Office of Disability Services may require additional information from a treating professional.  In order to request this information, the student must ask the treating professional to release that information.  This form, or a form provided by the treating professional, should be completed in order to authorize that information to be released to the Office of Disability Services.

Psychological Disabilities Diagnostic Report

The Psychological Disabilities Diagnostic Report (PDDR) is a form to be completed by a treating professional for psychological disabilities.  This form should be completed for a student requesting academic accommodations based on a psychological disability.

Medical Disability Documentation

Students who have a medical disability should have their treating professional complete this form to give pertinent information on the condition. 

Course Roster Form

The Course Roster form MUST be completed by students receiving academic accommodations EVERY SEMESTER.  This form allows the student to identify instructors and courses that should be notified of the student's approved academic accommodations.  Completion of this form is followed by an e-mail letter sent by ODS to those instructors indicating the accommodations that should be provided. 

Note that a student will not receive the approved accommodations if this form is not completed.  Additionally, it is the student's responsibility to communicate with the instructor at least two weeks in advance of the need for accommodations to discuss the implementation. If this is not done within 48 hours of the needed acccommodation, the instructor may not provide the accommodation.

Semester Calendar

The Semester Calendar is an organizational tool that can be used by any student to outline the long term or recurring assignments and obligations throughout the semester.  Using a student's course syllabi, all assignments and tests are recorded on the calendar on the date that they are due.  It is helpful to use different colors to represent each course.  This form can give a clear picture of "crunch" times which can then enable the student to plan in advance to meet those obligations.

Weekly Calendar

The Weekly Calendar is an organizational tool that can be used by any student to look at short term assisgnments or obligations and plan out how the student's time in the short term can most effectively be used.  By blocking out standing commitments, meal times, etc.  the student can then plan blocks of time to spend on completing work due in the short term.  This form is useful during the "crunch" times identified on the Semester Calendar.

Things To Do Form

The Things to Do Form is a place to record assignments, small tasks, etc. that you don't want to forget to do.  The form allows you to record long term goals as well as short term tasks.  In addition, there is a section to record progress on items that you have begun working on or are waiting for a follow-up response for.  Especially important is the column to record due dates, as this will help you to prioritze your Things To Do list.
